We will schedule your appointment as promptly as possible.  If you are experiencing pain or have an emergency situation, we will make every attempt to see you that day.  Please assist us in providing the following information at your first visit:
 
-      Current x-rays, if available

-      A list of your current medications

-      Your insurance card, or a completed insurance form
 
Please advise the receptionist if you have a medical condition that may require antibiotic premedication prior to your visit, such as a heart murmur, artificial heart valves and/or joints, or a history of rheumatic fever.
